Medical Forms

If your child requires any medication at school, please complete the following form and return to the School Office along with your child's medication. A copy of this form is available from the school. 

• Medication Authority Form

If your child has been diagnosed with Asthma or Anaphylaxis. Please consult with your child's doctor to complete a management form. These are required upon enrolment and need to be regularly reviewed to ensure your child is treated correctly in case of an emergency.

You are also required to provide correct medication to the school for your child when returning this form.
