How to Enrol

Step 1: Download Student Enrolment Form

To enrol at Warrnambool West Primary School, you will need to complete the Student Enrolment form in full and return it to our school office, along with supporting documentation. You can download it below or visit the School Office for a Student Enrolment Pack.

Step 2: Download Student Media Consent Form

When enrolling at Warrnambool West Primary School, you are required to complete a media consent form for your child. Please download and complete this form or contact the School Office for a hard copy.

Step 3: Enrolment Documentation

All student enrolments must be formalised by completing an enrolment form and providing copies of the following documentation:

• Proof of birth - Birth Certificate (English translated) or Passport
• Immunisation History Statement
• Medical Management Plans (Asthma and Anaphylaxis)
• Student and Parent Visas
• Latest School Report (if transitioning from Non-government Victorian school or International/Interstate school)

Step 4: Drop off Student Enrolment Information

All enrolment forms must be provided to the School Office along with supporting documentation. This can be done at any time while the school is open. We will be able to provide you with any other information you require at this time.
